Dani Maier, PMHNP, LLC reserves the right to review office policies at any time. Such revisions will be updated without advance notice to you and are effective immediately upon posting. I encourage you to check the office polices frequently.
Confidentiality
All information and records related to your care and treatment will be kept confidential and will not be disclosed without your written consent. State and Federal Laws require some exceptions:
When there is reason to believe you may be in danger of hurting yourself or others
When there is reason to believe that abuse or neglect of a child, elder, or someone with disability has occurred
When a court order is received or other situations required by law
For insurance billing purposes
When an emergency situation requires information be shared
Appointments and Treatment
Appointments must be scheduled either by telephone or email communication. The office will respond to appointment requests during business hours only. Appointment times vary in length related to individual needs, clinician recommendation, and as other treatment factors dictate. An initial evaluation will determine if Dani Maier, PMHNP is appropriate for your treatment needs prior to subsequent appointments being scheduled.
The length of treatment is determined by the nature of treatment, individual patient’s needs, response to treatment, and potential risks of treatment. This will be determined in collaboration with Dani Maier, PMHNP, LLC. Typically, a patient must be seen a minimum of every three (3) months to continue the provider-patient relationship. Sometimes after the initial assessment appointment(s) or in ongoing treatment, a referral to care with another provider or another level of care may be recommended as most appropriate for your individual care needs
Medication Management
All medications have the potential to cause side effects and potentially adverse events. Medications may also interact with other medications, including prescriptions and over-the-counter medications and herbs. It is therefore important that you update Dani Maier, PMHNP of any changes in your medications or supplements, but there is also no way of fully predicting potential effects of medication on a specific individual. The potential benefits, risks, and alternatives will be discussed prior to setting a treatment plan. Please also be advised that medications used in psychiatry are often prescribed “off-label.” This means that such medication may be used to treat/manage symptoms other than those for which it was originally approved by the FDA. This will be discussed during treatment planning.
Prescription Refills
Dani Maier, PMHNP, LLC will write prescription refills at your regularly scheduled appointments. It is your responsibility to schedule and attend follow-up appointments as determined in your treatment plan so you do not run out of medications. If an exception occurs, please request a refill via the patient portal or telephone. Refills outside of appointments will be considered on a case by case basis. Please allow a minimum of 5 working days before you will run out of medication. Refill requests will not be processed after hours, on weekends, or on holidays
Services Not Provided
Dani Maier, PMHNP, LLC does not provide:
Court evaluations or testimony including but not limited to child custody, workers compensation, or criminal cases.
Disability Evaluations outside of existing provider-patient relationships
Emotional support animal letters
Payment and Billing
Fee schedule charges are based on length, complexity, and type of service provided. You may find the fee schedule under the billing tab on Dani Maier, PMHNP, LLC’s website.
Payment in full (or copay/coinsurance) is due at the time of service. Dani Maier, PMHNP, LLC accepts major credit cards. Payment of any outstanding balance must be made within 60 days or by other arrangement with Dani Maier, PMHNP, LLC. Outstanding balances older than 90 days may be subject to a collections agency. Failure to make payments may result in discontinuation of services and/or may be turned over to an outside collection agent.
You are responsible for determining the details of your insurance benefits, including deductibles, copays, or coinsurance, pre-authorization, and network coverage. If you request or agree to a service for which your insurance company or its agent deems patient responsibility, then you assume responsibility for paying the entire balance. Insurance companies often request treatment information which would require release of confidential treatment information before payment is made
Additional Fees
Services provided outside of appointment times by Dani Maier, PMHNP are billed at $30 per 15 minute increment. This includes phone calls, other communication with you the patient, medication authorization, letters, or communication with collateral contacts. Please note, insurance does not typically pay for these services.
Declined credit cards will result in a $50 fee in addition to the owed balance.
Missed Appointments and Cancellations
Maintaining appointments is the responsibility of the patient. If you need to cancel or reschedule your appointment, please call, email, or notify the office via the patient portal 48 hours prior to the scheduled appointment time. If an appointment is missed with less than 24 hours notification (telephone message may suffice) a fee of $75 will be charged. If you miss your appointment without notifying our office by phone at least 24 hours in advance you will be charged $125. If you arrive more than 15 minutes late to an appointment you will be asked to reschedule and charged for a late cancellation. If Dani Maier, PMHNP is late, she will make up the missed time. Fees for missed or late cancelled appointments are not reimbursable by insurance companies. If cancelling appointments or no-shows become a regular occurrence you may be notified of risk for discontinued treatment
Termination of Treatment
Discontinuation of treatment may occur when/if:
Requested by the patient
The goals of treatment have been met
A patient has been referred to another clinician or program who may be of better service
Payment is not received
A patient or family member/significant-other are hostile or aggressive
Dani Maier, PMHNP, LLC reserves the right to terminate treatment with a patient who is not actively participating in treatment.
A patient will be considered no longer actively involved in treatment when/if:
They “no-show” for an appointment and do not contact Dani Maier, PMHNP, LLC within 30 days
They “no-show”/“late cancel” for two consecutive appointments
Otherwise not engaged in treatment
Communication
Please communicate with Dani Maier, PMHNP, LLC via telephone, secure email, or via the patient portal. Messages will be checked at least once per regularly business day (Dani Maier, PMHNP, LLC operates on Mondays, Wednesdays, and Thursdays), more often during business hours and an attempt to return all messages will be made within 24-48 hours. Dani Maier, PMHNP, LLC does not use texting as a form a communication
Dani Maier, PMHNP, LLC does not offer 24 hour on-call coverage. In the case of an emergency, call 911 or go to the nearest emergency department. In the event of a crisis in which you need assistance before Dani Maier, PMHNP is able to return your call you may also contact:

TELEHEALTH SERVICES
Currently, Dani Maier, PMHNP, LLC is providing services utilizing telephone and video platform for providing remote mental health/psychiatric services. Due to the COVID-19 pandemic, most insurance companies have expanded coverage of and reimbursement for telehealth services. There is a wide variation in definition, coverage and allowable uses of telehealth services among different insurance companies. For this reason, we ask that you gather as much information from your insurance company as you can prior to engaging in telehealth services with Dani Maier, PMHNP, LLC. This is not a guarantee your insurance will pay for services. If you elect to use telehealth services and your insurance does not pay, you will be billed for the services rendered. If at some point, services provided by Dani Maier, PMHNP, LLC include in-person face-to-face appointments, you will be informed of this option of care.
The confidentiality laws that protect the privacy and confidentiality of medical information also apply to telehealth. All video sessions will be conducted through a secure and HIPPA complaint platform and will not be recorded. Please do not record your sessions without consent from Dani Maier, PMHNP. Dani Maier, PMHNP will ensure she is located in a secure and private location. You are responsible for selecting a private location that does not include others and agree to inform Dani Maier, PMHNP if others are present during the session. You are responsible for the configuration of any electronic equipment used and responsible to ensure the proper function of all electronic equipment before your session. You must also be an Oregon or New Mexico resident and located in the State of Oregon or New Mexico during the time of the appointment to be eligible for telehealth care.
Dani Maier, PMHNP, LLC reserves the right to discontinue and reschedule a telehealth appointment if:
Patient Privacy is compromised.
Patient Electronic equipment does not function properly to complete an appointment
You are not located in the state of Oregon or New Mexico
If a telehealth appointment must be discontinued and rescheduled due to the above reasons, you may be charged the equivalent of a late cancellation.
In the event our audio or video session is disrupted prematurely and Dani Maier, PMHNP believes you or someone else may be in danger she will take necessary and appropriate actions. These may include reaching out to your emergency contact on file, requesting a welfare check from a local law enforcement agency, or calling 911
