Initial Appointment Request

Dani Maier, PMHNP treats patients 16 years old and older. To request an initial assessment, please complete the Initial Appointment Request form. Once received, it will be reviewed and you will be contacted within two weeks via the method indicated on the form regarding the request. Completing an appointment request form does not guarantee an appointment.

Before your visit

Please read completely the Office Policies and make sure the Office Policies form is signed and submitted before your initial appointment.

Consider completing a Release of Information for any current or past providers to improve continuity and coordination of care (this could include psychiatric or primary care providers). Over the course of treatment, you may also be asked to complete a release.

Make sure you have a complete list of current medications (including prescriptions, over the counter, and herbal supplements) at the time of the appointment. As able, it is helpful to have a complete list of other medications and treatments tried as well.

Call your insurance carrier to confirm benefits and make sure you understand any costs associated with treatment.

how to Sign in to the patient portal and telehealth appointments

At this time, all appointments will be completed via telehealth. You must have synchronous video capabilities (device with camera and internet connection) to receive care. Once you are scheduled, you will be sent a link to sign up for the Patient Portal where you will be able to complete visits, send secure messages, and pay bills.

You will be prompted to enter your username and password (which you created when registering) to sign in. Once in the Patient Portal, please check in to your appointment at least 5 minutes early. Once the video visit has been initiated you will be able to enter the appointment.
